What Is A Pharmacy Medicine Use Review?
A Medicine Use Review (MUR) is an Advanced Service of the New Pharmacy Contract in England & Wales. The aim of the service is to:
- Establish the patient´s actual use and understanding & experience of taking their medicines.
- Identify discuss & resolve poor or ineffective use of medicines.
- Identify side effects & drug interactions that may affect patient compliance.
- Improve the clinical effectiveness & cost effectiveness of prescribed medicines & reduce medicine wastage.
The review focuses on medicines use and is not a clinical review. The Pharmacist will not have access to your clinical notes but will know from their pharmacy patient medical record what medicines you take.
Who Will Deliver The Service?
Only accredited Pharmacists from premises accredited with the PCT can perform a medicine use review. The review will normally be conducted in a consultation area within the pharmacy. The Pharmacist conducting the review must have undertaken a competency assessment before providing the service.
Who Can Have A Medicine Use Review
Any patient who regularly takes multiple medicines & those with long term conditions. A review can be carried out every 12 months and the review can either be planned (in which case you must have been using the pharmacy for dispensing your medicine for the previous 3 months) or by intervention. In this case a problem may be highlighted as part of the dispensing process (the 3 month rule will not apply to intervention MUR’s).
What Will The Review Involve?
The pharmacist will ask you a series of questions in relation to what you think each medicine is for, when you take it, whether you are complying with the prescribed instructions and whether you are suffering any side effects.
The review will be recorded on a national standard form. A copy will be retained in the pharmacy, a copy will be given to you and a copy sent to your GP. There may be action points for the GP dependent upon the outcome of the review.
